Personal Information Collection Statement
1. The personal information collected from you will be used for the purpose of processing your application and managing your membership.
2. Please note that it is mandatory for you to provide personal information in Part A of this Application Form. In the event that you do not provide such personal information, we may not be able to proceed with your membership application.
3. As for Part B, the provision of requested information is solely voluntary. However, such provision would greatly assist the Federation in processing your application.
4. Your signature in this application form may be used by us for the purpose of identity verification for managing your membership and operation of the Federation.

Privacy Policy Statement
1. We pledge to comply with the requirements of the Personal Data (Privacy) Ordinance. In doing so, we will ensure compliance by our staff with the strictest standards of security and confidentiality.
2. Your personal information collected and held by us will be used for the purpose of managing your membership and furtherance of the objects of the Federation only.
3. We will not provide your personal information to third parties for direct marketing or other unrelated purposes without your consent.
4. Your personal information will be deleted permanently either within 3 months after the rejection of your application, or 1 year after the termination of your membership.
5. You may need to present your Hong Kong Identity Card to verify your residency status. However, we will not record your Hong Kong Identity Card number.
6. All your personal information received in hard copies will be converted to digital format for better management, upon which all hard copies would be destroyed. Only relevant employees or agents of the Federation would have access to your personal information on a “need-to-know” basis through password protected computer files.
7. You have the right to request access to and correction of information held by us about you. If you wish to access or correct your personal data, please contact our Membership Director via e-mail at laiking88@gmail.com
